Importance of collaborative care and patient support services
In this presentation we will discuss the prevalence and impact of age-related macular degeneration (AMD) in Australia and how the MDFA supports Australians living with macular disease. We will review how MDFA supports patients through education and evidence-based resources, peer support and facilitating access to low vision services. MDFA works collaboratively with Optometrists to assist patients to make the dietary and lifestyle modifications that may lead to reduced risk of AMD progression, improved treatment adherence and improved patient outcomes.
